Visiting Hospice Program
Being diagnosed with a life limiting illness can be overwhelming for both the individual diagnosed, and their loved ones. Hospice can help; we can help to navigate this difficult journey. Hospice compliments other health care services by providing support to the client wherever they are residing, whether that be in their own home, a nursing home, a retirement home, hospital, or in our 3-bed Hospice House.
Anyone can refer. Upon receiving a referral, a Coordinator will meet with you and/or your caregivers to determine how we can help. Trained Hospice Volunteers function as extended family members. They can provide companionship by talking, listening, or just sitting quietly. Volunteers can also assist the caregiver by providing them with time to run errands, participate in their own activities or tend to their self care, OR, they can run errands for you, such as picking up medication or groceries, by preparing a small meal, or assisting with personal care.

If the individual and family would like a volunteer and feel they would be of benefit, the coordinator will work to match them with a volunteer in their area. If a volunteer is not needed, other services will be explained and offered and the hospice coordinator will follow up to check in on a monthly basis with a phone call, or at the patient/family request.
The Heart of Hastings Hospice accepts referrals from anyone; individuals, family members, friends, visiting nurses, physicians, clergy, or home care workers. Use our contact form or call (613) 473-1880 x107 to speak with a Coordinator about our Community Hospice program.

Upon receiving a referral, our coordinator will contact you to set up a time to visit you/your family.
Assessment
​
Our coordinator will meet you in your home to discuss our programs, assess your needs and eligibility for service, ensure the environment is safe for our volunteers and get to know you a little better.
Initial Visit
​
Our coordinator will meet with you and your new volunteer in your home. During this time, we will assist the client and volunteer to "break the ice" and get to know each other and the care needs.
Ongoing Visits
​
Future visits are left to develop organically between the client and volunteer, be that on a regularly scheduled basis, or on an as needed basis.
