Ontario Palliative Care Network
A partnership of community stakeholders, health service providers and health systems planners who are developing a coordinated and standardized approach for delivering hospice palliative care services in the province. Funded by the Ministry of Health and Long-Term Care to help deliver on Ontario’s commitment to palliative care.
Work is person-centred, focusing on supporting the provision of quality hospice palliative care for all Ontarians regardless of their age or disease type.

South-East Regional Palliative Care Network
With the introduction of the OPCN there is now a shared leadership and accountability for the delivery of quality hospice palliative care between the South East Regional Cancer Program (SERCP), and the South East Local Health Integration Network (LHIN). Although executive oversight sits with the South East LHIN CEO and SERCP RVP, the South East Regional Palliative Care Network (RPCN) is a partnership of all of our regional community stakeholders, health service providers, health systems planners, patients, families and caregivers who have role in hospice palliative care in our region. As a community of partners the South East RPCN is committed to improved access and equity in hospice, palliative and end-of-life care.
